Community Contact Points

Community Contact Points are trusted people, places, and connections that help residents find their next step.

Not everyone knows where to start when they need support, information, forms, resources, or someone to talk things through with.

Grassroots Westmoreland works to build low-barrier pathways so people are not left trying to navigate everything alone.

How Contact Points Help

Community Contact Points help make support easier to find and easier to understand.

Listen & Identify Needs

A Contact Point may help someone talk through what they need, what feels urgent, and what kind of support might make sense.

Share Information

Contact Points may help connect people with forms, resource links, program information, community updates, or next steps.

Connect to Support

When possible, Contact Points help connect residents with Grassroots Westmoreland, partner resources, or trusted community support.

What This May Look Like

Community Contact Points may help in simple, practical ways.

That might include helping someone find the Needs Form, sharing current resource links, connecting someone to Grassroots Westmoreland, helping explain next steps, pointing someone toward a partner organization, or sharing updates about community support.

Contact Points are not expected to solve everything alone. Their role is to help people find a next step, not carry the whole system by themselves.

What Contact Points Are Not

Community Contact Points are not case managers, emergency responders, legal representatives, or crisis services.

They do not promise that every need can be met, and they are not responsible for fixing everything alone.

Their role is to help people find information, connection, forms, resources, or a next step when possible.

Need help finding a next step?

Use the Needs Form or contact Grassroots Westmoreland if you are not sure where to start.
